29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) RR Drilling LLC, 1615 Platte St, Denver, CO 80204: On or about 4/11/2022, the employer did not ensure that each employee was trained in the recognition and avoidance of hazardous trench conditions prior to working in a trench at a depth greater than five feet, with vertical walls, cut in type b soil. Employees were exposed to cave-in and crushing hazards without the ability to recognize the hazards or take actions to stop exposure or mitigate the hazards.

29 CFR  1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  a) RR Drilling LLC, 1615 Platte St, Denver, CO 80204: On or about 4/11/2022, the employer did not ensure that employees had a safe means of egress from a trench that was greater than five feet deep cut in type B soil. Employees entered the trench by jumping in and exited by scrambling up the walls with the assistance of others standing at the edge of the trench.  This condition exposed employees to cave-in and crushing hazards.

(a) RR Drilling LLC, 1615 Platte St, Denver, CO 80204: On or about 4/11/2022, the employer did not ensure workers were protected from the sluffing of spoils piles as the spoils pile was only six inches away from the edge of the trench. This exposed workers to stuck-by and crushing hazards.
