29 CFR 1910.134(f)(2):Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter: At Holyoke Medical Center at 575 Beech Street, Holyoke, MA: On or about April 11, 2022, and at times prior, the employer did not perform initial fit tests or annual fit tests on all employees required to wear respirators. The employer required employees to wear tight fitting respirators to protect against the SARS-CoV-2 virus, while performing custodial, cleaning, and disinfection duties, while exposed to suspected and confirmed positive COVID-19 patients.

29 CFR 1910.134(g)(1)(i)(A):Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: Holyoke Medical Center at 575 Beech Street, Holyoke, MA: On or about April 11, 2022, and at times prior, the employer permitted employees who had facial hair that comes between the sealing surface of the facepiece and the face to wear tight fitting respirators. The employer required employees to wear tight fitting respirators to protect against the SARS-CoV-2 virus during housekeeping duties, while exposed to confirmed positive COVID-19 patients.
