Round Tree Olathe Kansas, employees engaged in residential framing activities to build the porch, using the GEHL DL-6H 42, machine number 20440, serial number 3H42300828440 Dyna Lift Rough Terran Forklift Truck to support a job-made wooden platform. The platform was not designed or approved by the manufacturer of the Dyna Lift Rough Terran Forklift. The platform was not secured to the forklift. Employees were exposed to fall hazards of approximately 11 feet 1 inch from the working surface to the ground below in that fall protection was not utilized, and there was no safety railings around all sides of the platform.
