29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  Jobsite located at 14465 S. Hauser St. Olathe Kansas, employees engaged in residential stucco/masonry activities were exposed to electrical shock and fire hazards in that the orange extension cords used to supply power to the Bauer Mixer bit to stir the cement was not equipped with adequate strain relief.  As a result, the extension cord showed signs of damage.  The insulation had pulled away from the plug, exposing the conductors.
