29 CFR 1926.405(g)(2)(iv): Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. At jobsite located at 11854 S. Kenton St, Olathe KS, employees engaged in framing activities were exposed to electrical shock and fire hazards in that the flexible cord protective layer was pulling away from connector.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Employees were exposed to fall hazards. At the job site located at 11854 S. Kenton St, Olathe, Kansas, employees were exposed to fall hazards of approximately 11 feet while accessing the roof surface. The aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof nor was it secured to the structure.
