At jobsite located at 11854 S. Kenton St, Olathe KS, employees engaged in framing activities were exposed to electrical shock and fire hazards in that the flexible cord protective layer was pulling away from connector.

Employees engaged in residential framing activities in that the aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof nor was secured to the structure.
