Sub-Contractor employees engaged in concrete masonry activities were exposed to fall hazards of approximately 20-feet while working at are near wall openings with sagging/falling netting and no guardrails installed, on or about 4/11/22.

Location: 618 Pavonia Ave., Jersey City, NJ 07306 -2nd Level a) The employer did not ensure insure frequent and regular inspections of the job sites, materials, and equipment were made by competent persons designated by the employer. Employees and Sub-contractor employees were exposed to fall hazards of approximately 20-feet while walking around and performing mortar pouring and bricklaying operations within 4 feet of the building open edges without a secured guardrail system and/or a safety net system around the exterior of the building, on or about 4/11/22.
