29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about April 12, 2022, employees performing framework on leading edge of a commercial building roof were not protected from falls. This condition exposed employees to fall hazards from heights of 6 feet or more from the ground.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about April 13, 2022, the side rails of a portable extension ladder used to access the building roof did not extend at least 3 feet above the upper landing surface. This condition exposed employees to fall hazards from heights of six feet or greater.
