On or about April 11, 2022, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing the employees to hazards associated with eye injuries. Mark Kyrouac dba Mark Kyrouac was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1503579, citation number 1, item number 1 and was affirmed as a final order on January 19, 2021, with respect to a workplace located at 2341 W. Walton St., Lot 33 Peacock Pl. Manteno, IL 60950.

On or about April 19, 2022, at the above addressed jobsite, employees performing residential roofing operations, nine (9) feet seven(7) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.
