29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about April 11, 2022, at the above addressed jobsite, employees using pneumatic nail guns were not wearing safety glasses, thereby exposing the employees to hazards associated with eye injuries.  Segundo F. Cazho Quizhpi dba Carlos Cazho Construction was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1509848, citation number 2, item number 1, and was affirmed as a final order on May 26, 2021, with respect to a workplace located at Lot 94 Coquille Dr.  in Frankfort, Illinois.  Segundo Quizhpi dba Segundo Quizhpi was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1488922, citation number 1, item number 1, and was affirmed as a final order on November 16, 2020, with respect to a workplace located at Lot 60 Primrose St. in Lockport, Illinois.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about April 11, 2022, at the above addressed jobsite, an employee performing residential roofing operations, eleven (11) feet five (5) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  Segundo F. On or about April 11, 2022, at the above addressed jobsite, an employee wearing a personal fall arrest system while performing residential roofing operations was not attached to the lanyard and rope by the D-ring in the center of the employee's back near shoulder level, thereby exposing the employee to fall hazards.
