29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On April 11, 2022, an employee operating a skid steer loader was clearing some brush near a dry creek when they backed down the creek embankment, causing the loader to flip over. As a result of the incident, the employee was hospitalized and the employer did not report this to the local Occupational Safety & Health Administration office within 24-hours.
