29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  Production Area; on or about April 12, 2022, and times prior to, employees were working with chemicals to include, but not limited to Methylene Chloride, Aluma Black (containing selenious acid, phosphoric acid, fluoboric acid); and Ferric Chloride (containing hydrochloric acid), acetone, aluminum alloy, metal finish, etc.  The employer had not developed a PPE assessment.  Note: The PPE hazard assessment needs to be certified in writing in accordance with 1910.132(d)(2).

29 CFR  1910.151(c):29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  Production Area; on or about April 12, 2022, and times prior to, employees were exposed to skin and face burn hazards while working  with and near chemicals such as, but not limited to, Aluma Black, Ferric Chloride, and methylene chloride, classified as corrosive.  The eye wash station provided was not adequate to flush the body or both eyes simultaneously in the event of a splash to the eyes and body.
