29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Residential roof structure, various locations; An employee performing framing work was not protected from falls while working at heights ranging between 9 feet, 8 inches to 21 feet, 5 inches above ground level.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  (a) Residential roof structure, various locations; When an employee was exposed to fall hazards ranging between 9 feet, 8 inches to 21 feet, 6 inches above ground level, the employer did not prepare a written certification record to verify training was provided on applicable topics, including but not limited to, fall hazard recognition and appropriate mitigation methods.

29 CFR  1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval:  Note:  If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly.   (a) North side of jobsite, ground level; A steel platform attachment approximately 72 inches wide by 131 inches long placed on the 48 inch forks of a Sky Trak Model 6036C rough terrain forklift and used to raise/lower tools and materials was not approved by the forklift manufacturer prior to use to account for factors including but not limited to, weight, load capacity and load center.
