(a) North side of jobsite, ground level; A steel platform attachment approximately 72 inches wide by 131 inches long placed on the 48 inch forks of a Sky Trak Model 6036C rough terrain forklift and used to raise/lower tools and materials was not approved by the forklift manufacturer prior to use to account for factors including but not limited to, weight, load capacity and load center.
