29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s):   a.  1010 Hamilton Blvd., South Plainfield, NJ - Roof  The employer did not provide effective training to employees in the recognition of unsafe conditions and methods to avoid such hazards.  Employees engaged in residential roofing activities were exposed to hazards that included, but were not limited to, fall hazards from the roof while working and not utilizing fall protection, struck by hazards by debris and/or materials falling from the roof, and the use of ladders that were not properly extended to ensure safe access.   Violation occurred on or about April 13, 2022

Employees engaged in the clean-up of residential roofing materials, working directly below employees disposing of roofing materials from the roof, were not protected by use of hardhats.
