Abriendo Ave, Pueblo CO 81004: On or about April 12, 2022, and at times prior, the employer did not correctly document two injuries on the OSHA 300 form provided to the compliance officer. Two cases on the 2021 OSHA 300 form did not have the correct number of days on restricted duty/job transfer documented.
