29 CFR  1910.28(b)(7):  The employer did not ensure that each employee on a walking-working surface near an opening, including one with a chute attached, where the inside bottom edge of the opening is less than 39 inches (99 cm) above that walking-working surface and the outside bottom edge of the opening is 4 feet (1.2 m) or more above a lower level is protected from falling by the use of one of the systems listed in 29 CFR 1910.28(b)(7)(i)-(iv).    a) 72-inch Steel Slitter Line, Coil car pit: On or about 04/13/2022, and at times prior, the employer exposed employees to fall hazards of approximately 59-inches to the bottom of the pit, in that a fall protection system was not provided along all open sides of the 5-ft. wide by 9-ft, long pit.

29 CFR  1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements.    a) 72-inch Steel Slitter Line, Wooden steps to Slitter platform: On or about 04/13/2022, and at times prior, the employer exposed employees to fall hazards of approximately 46-inches to the lower concrete surface, in that stair rails were not provided along the open sides of the wooden stairway provided to the Slitter platform.

29 CFR  1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition:  (a) Collier Metals LLC Plant: On or about 04/13/2022, the employer did not ensure that the Komatsu (Model FD45T-8) Forklift Truck which was in need of repair due to defective or non-operable horns, broken windshield, and torn operator's seat was not taken out of service until it had been restored to safe operating conditions. This condition exposed employees to a struck-by hazard.

29 CFR  1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):   a) 72-inch Steel Slitter Line, Solenoid Valve: On or about 04/13/2022, and at times prior, the employer exposed employees to electrical/burn hazards near the coil cart pit area, in that employees were exposed to 120v electrical wiring which provided power to a solenoid which was not provided with a cover or enclosure to prevent accidental contact by employees engaged in metal slitting operations.

29 CFR  1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:   a) 72-inch Steel Slitter Line, Portable Hydraulic Unit used at Re-coiler drum: On or about 04/13/2022, and at times prior, the employer exposed employees to electrical/burn hazards, in that energized flexible cords used to provide power to the hydraulic unit had torn and frayed wires, missing ground pin and improperly repaired casing with masking tape used for repairs. The employer failed to remove these cords from service until repaired.
