29 CFR  1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.  At the jobsite located at12609 169th Street, Overland Park,  employees engaged in residential roofing / framing activities using a standard hammer were exposed to struck-by hazards in that eye protection was not provided.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall protection   At the jobsite located at12609 169th Street, Overland Park, in setting braces at new home construction build, employee engaged in construction activities were exposed to fall hazards of 11' inside the structure and 30 ' outside  the structure to the ground level below in that a guardrail or fall protection system was not in place.

29 CFR  1926.1053(b)(13): The top or top step of a stepladder was used as a step    At the jobsite located at 12609 169th Street, Overland Park, in having employees get to upper crossbeam, employees were exposed to walking, working surface hazards by using a 6' A Frame ladder that was too short to provide required elevation over necessary area, and required employees to stand on top part of ladder.
