On or about April 12, 2022, at jobsite located in 150 Avon Road., Elmhurst, IL 60126, employees were exposed to eye injuries while engaged in roofing activities operating a pneumatic nail guns. Eye protection was not being utilized while employees engaged in this activity.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about April 12, 2022, at jobsite located in 150 Avon Road., Elmhurst, IL. 60126, an employee was exposed to fall hazards in excess of 20 feet while engaged in residential roofing activities. Fall protection was not being utilized while performing roofing activities. The employee was wearing a harness but not tied-off to lanyard.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grab rail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. On or about April 12, 2022, at jobsite located in 150 Avon Road., Elmhurst, IL 60126, the portable ladder to access the second level did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to fall hazards. Abatement certification is required for this item in accordance with 29 CFR 1903.19(c)
