29 CFR 1915.81(a)(4): The employer did not maintain easy and open access to each fire-alarm box, fire-call station, fire-fighting equipment, and each exit, including ladders, staircases, scaffolds, and gangways: a) McFarland Dredge: On or about 03/22/2022, the employer failed to ensure that access onto the scaffold ladder was easy and open in that employees were required to step through or climb over a guardrail system to access the scaffold and ladder. No abatement certification or documentation required.

29 CFR 1915.81(b)(1)(iv)(A): The employer did not place each hose and cord above the walkway: a) McFarland: On or about 03/24/2022, the subcontractor employees did not place hoses and extensions above the walkway in the hoses and extension cords used in descaling the hopper were placed on the deck of the walkway above the hopper. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
