29 CFR 1910.134(e)(6)(i): The employer did not obtain from the physician or other licensed health care professional (PLHCP) a written recommendation regarding the employee's ability to use the respirator that includes only the following information: Any limitations on respirator use related to the medical condition of the employee, or relating to the workplace conditions in which the respirator will be used, including whether or not the employee is medically able to use the respirator; and a statement that the PLHCP has provided the employee with a copy of the PLHCP's written recommendation: a) In the facility, on or about April 14, 2022 - The employer did not obtain a written recommendation that contained only the requirements under 1910.134(e)(6)(i)(A)-(C). The PLHCP's recommendation was attached and included as part of the medical questionnaire, returned to the employer, and did not contain a statement that the PLHCP provided the employee with a copy of the PLHCP's written recommendation. The employer required employees to wear respirators while proving care to suspected and confirmed positive COVID-19 patients.
