At the residential construction site located at 8516 Booth Ave, KANSAS CITY, MO 64156, employee was operating/working from aerial lift without being properly anchored. This exposed the employee(s) to be catapulted from the aerial lift causing fall injuries. This was documented on or about 04/14/2022.

At a residential construction site located at 8516 Booth Ave, KANSAS CITY, MO 64156, Employees were exposed to fall hazards while conducting water proofing activities without the use of fall protection at a height greater than 6 feet. The employees were working 2 stories high on a steep residential roof without any sort of fall protection system. This was documented on or about 04/14/2022.
