On or about April 14, 2022, at an excavation site located at NW 122nd and Legends Blvd. in Oklahoma City, OK., an employee was observed working in an excavation greater than 5  feet in depth that did not have stepping or other protective devices in place, exposing the employee to struck-by hazards.
