29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. Beckett Springs Hospital located at 8614 Shepherd Farm Drive, West Chester, OH 45069: a) On or about April 14, 2022 the employer did not provide annual fit tests to all employees required to wear a respirator. Psychiatric Nurse Practitioners and Therapists working on Cedars were required to wear tight fitting N95 filtering facepiece respirators to protect against SARS-CoV-2 on or about January 14, 2022, while conducting psychiatric assessments of confirmed COVID-19 positive patients.
