Roof area at a worksite located at 2975 Rexford Street in Jacksonville, Florida 32205: On or about April 13, 2022, the employer exposed an employee to eye injury hazards, in that eye protection was not utilized by the employee who was operating pneumatic nail gun to nail down sheets of plywood, while performing framing operations. b. Inside of home, front room at a worksite located at 2975 Rexford Street in Jacksonville, Florida 32205: On or about April 13, 2022, the employer exposed an employee to eye injuries hazards, in that eye protection was not utilized by the employee who was operating a hand held saw to cut wood.

On a 5:12 pitched roof, at a worksite located at 2975 Rexford Street, in Jacksonville, Florida 32205: The employer exposed employees to fall hazards of 8 - 15 feet to the ground below while engaged in decking activities in that the employer failed to ensure fall protection was utilized.
