29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. Roof area at a worksite located at 2975 Rexford Street in Jacksonville, Florida 32205: On or about April 13, 2022, the employer exposed an employee to eye injury hazards, in that eye protection was not utilized by the employee who was operating pneumatic nail gun to nail down sheets of plywood, while performing framing operations.  b. Inside of home, front room at a worksite located at 2975 Rexford Street in Jacksonville, Florida 32205: On or about April 13, 2022, the employer exposed an employee to eye injuries hazards, in that eye protection was not utilized by the employee who was operating a hand held saw to cut wood.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 5:12 pitched roof, at a worksite located at 2975 Rexford Street, in Jacksonville, Florida 32205: The employer exposed employees to fall hazards of 8 - 15 feet to the ground below while engaged in decking activities in that the employer failed to ensure fall protection was utilized.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:   a. Front of the home at a worksite located at 2974 Rexford Street,  in Jacksonville, Florida 32205: On or about April 13, 2022, the employer exposed employees to fall hazards in that, the 10-foot aluminum  extension ladder side rails did not extend 3 feet above the landing.
