29 CFR 1904.41(a)(2): On April 15, 2022, Laurate Terrace Operations, LLC, dba Terrace Nursing Home did not electronically submit information from the OSHA Form 300A Summary of Work-Related Injuries and Illnesses for calendar year 2021, to OSHA or OSHA's designee.  The establishment had 62 employees at any time during calendar year 2021 and was classified within an industry listed in Appendix A to Subpart E 1904 - Recording and Reporting Occupational Injuries and Illness - for NAICS code 623110 but did not electronically submit information from the OSHA 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form which was March 2, 2022.

29 CFR  1904.32(b)(6):29 CFR 1904.32(b)(6)  The annual summary was not posted no later than February 1 of the year following the year covered by the records, and kept in place until April 30:  (a) On April 15, 2022, OSHA Form 300-A, summary form for calendar year 2021 was not posted  on any of the facilities posting boards, including: the community area, the elevator bulletin boards, the employee lounge, or the maintenance area, places where notices to employees are customarily posted.  In accordance with 29 CFR 1903.19 (d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
