29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On April 12, 2022, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 20 feet above the ground.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

The area around the bottom of the ladder was not kept clear. 29 CFR  1926.1053(b)(15):Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:  a) On April 12, 2022, a competent person did not inspect ladders used by employees at 1400-1416 Bridgeport Dr., Mount Prospect, IL 60056. Ladders used at the site had multiple broken and damaged rungs.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)

29 CFR  1926.1053(b)(16):Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  a) An approximately seven foot portable ladder used to access a second story roof located at 1402 Bridgeport Dr., Mount Prospect, IL 60056 had a broken rung and a bent rung exposing employees to a fall hazard.  b) An approximately 24 foot extension ladder was used to access a second story roof located at 1414 Bridgeport Dr., Mount Prospect, IL 60056 had a bent rung exposing employees to a fall hazard.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
