29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) ADB Companies, LLC, 1615 Platte St, Denver, CO 80204: On or about 4/11/2022, As the controlling employer, ADB Companies, LLC did not ensure that employees of subcontractor RR Drilling, LLC, had a safe means of egress from a trench that was greater than five feet deep cut in type B soil. Employees entered the trench by jumping in and exited by scrambling up the walls with the assistance of others standing at the edge of the trench. This condition exposed employees to cave-in and crushing hazards.
