29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Exterior, second floor:  An Employee was exposed to head injuries while working below framing activities. The employee was not protected by a hard hat or protective helmet, on or about 4/13/22  b) Roof floor: Employees were exposed to head injuries while framing activities. The employees were not protected by a hard hat or protective helmet while installing roof rafters, on or about 4/13/22

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                            a) Exterior, Roof Level of house: An employees was exposed to eye/face injuries while using pneumatic nail guns to install rafters. Employee was not protected with any type of eye/face protection, on or about 4/13/22.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.
