29 CFR 1926.453(b)(2)(iv):Employee(s) working in an aerial lift were not standing firmly on the floor of the basket. At the commercial construction site located at 7441 NW Old Tiffany Springs Rd, KANSAS CITY, MO 64153, employee was standing on the mid rail of an aerial lift while installing siding at extended heights. This exposed the employee to fall hazards of 10 plus feet to ground level. This was documented on or about 04/14/2022.

29 CFR 1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. At the commercial construction site located at 7441 NW Old Tiffany Springs Rd, KANSAS CITY, MO 64153, employee was working in an aerial lift and not tied off to the platform. This exposed the employee to fall hazards over 10 feet. This was documented on or about 04/14/2022.
