At a commercial construction worksite located at 901 Barkley Pkwy, KC, MO 64120, employee was operating/working from an aerial lift with out being properly anchored to the basket. This exposed the employees to being catapulted from the aerial lift and falling to the ground approximately 15 feet. This was documented on or about 04/14/2022.
