DBA Brookdale The Heights, located at 2121 Pinegate Street Houston, TX 77008: On or about January 8, 2022, the employer did not provide a fit test to all employees required to wear respirators. The employer required employees to wear N95 filtering facepiece respirators to protect against the SARS-CoV-2 virus when other employees were confirmed COVID-19 positive.
