Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b). On or about April 19, 2022, at the residence located at 716 Foote Street in Seymour, Wisconsin; employees of Overhead Solutions Inc, engaged in residential roofing operations, were exposed to fall hazards greater than 6 feet and were not provided with fall protection.
