29 CFR  1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to the  items of this section:     On and about April 20, 2022,  the employer developed procedures for a 250 Ton Hydraulic Press (noted as Press #2) did not clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy including the follow items:    - A specific statement of the intended use of the procedure (per 1910.147 (c)(4)(ii)(A), -Specific procedural steps for shutting down, isolating, clocking, and securing the electrical and pneumatic energy sources (per 1910.147 (c)(4)(ii)(B), -Specific procedural steps for placement of lockout or tagout devices on the electrical and pneumatic energy sources (per 1910.147 (c)(4)(ii)(C),  -Specific requirements for testing the press to determine and verify the effectiveness of the lockout, tagout, or other energy control measure placed on the press (per 1910.147 (c)(4)(ii)(D)).

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On and before March 20, 2022, the employer did not conduct a periodic inspection of the energy control procedures, including but not limited to procedure for the 250 Ton Hydraulic Power Press.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On and before March 20, 2022, an employee was exposed to accidental cycling of a 250 Ton Hydraulic Power Press while retooling the press. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the service work {per the 1910.147(d)(2) requirements. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
