29 CFR 1910.134(f)(2):Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: a. Fairfield Medical Center located at 401 N. Ewing Street, Lancaster, Ohio 43130: On or about April 20, 2022, the employer did not provide an annual respirator fit test for two employees who were required to wear a N95 respirator including, but not limited to, 3M 1860 N95 respirator to protect employees from exposure to SARS-CoV-2 (the virus that causes COVID-19 disease).
