29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Workers were exposed to fall hazards at a worksite located at 2950 Summit St, Kansas City, MO 64108: This was demonstrated when employees were working from aerial boom lifts (JLG 400S - 95962) without using a tethering device, or personal fall protection. The lifts were last used on or about April 8, 2022.

29 CFR  1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems:  Workers were exposed to fall hazards at a worksite located at 2950 Summit St, Kansas City, MO 64108: This was demonstrated when workers were working above 25 feet placing trusses on a commercial building. Workers were without a tethering device, personal fall protection, or fall protection systems. The area was last accessed on or about April 8, 2022.
