29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Third Floor - Employees were exposed to a fall hazard of 30' 9" while installing plywood decking, on or about 04/20/2022.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.  ABATEMENT DOCUMENTATION REQUIRED

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards.   Job Site - Employees were not provided training on how to recognize fall hazards in the workplace as evidenced by fall hazards observed during an inspection that included a fall of over 30 feet, on or about 04/20/2022.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR  1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area:  a) Job Site - Where employees were using gasoline to fill up the Honda air compressor and  applying Titebond subfloor adhesive to the top of floor joists prior to installing plywood decking hazard communication training was not provided to the employer, on or about 04/20/2022.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
