29 CFR 1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. During the week of March 27, 2022, at an establishment located at 201 East Hospital Road, in Eagle River, Wisconsin; in the Emergency Room department, N95 respirator(s) were worn while providing care to suspected and confirmed COVID-19 patients. The employer had not fit tested employees required to wear the N95s at least annually.

You must enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred. On or about April 20, 2022, at an establishment located at 201 East Hospital Road, in Eagle River, Wisconsin; the enter had not entered COVID-19 illnesses on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving a positive COVID-19 test. (a) On or about January 22, 2022, in MedSurg, the employer did not record a COVID-19 illness after receiving the information that a recordable illness occurred. (b) On or about January 25, 2022, in MedSurg, the employer did not record a COVID-19 illness after receiving the information that a recordable illness occurred. (c) On or about January 26, 2022, in MedSurg, the employer did not record a COVID-19 illness after receiving the information that a recordable illness occurred.
