29 CFR 1910.178(a)(4):Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly: Site- Employees were not protected from overloading, struck by, and fall hazards where the Crown SP 3400 Series, serial number 1A350125, order picker was operated with a Koke Inc., model OP-4896A-ERR, 8 foot platform attachment and did not have an identification plate which identified the capacity of the order picker with the attachment in place. The platform attachment displayed a capacity of 1500 pounds on the side of the platform which exceeded the maximum capacity of the order picker with the platform attached.

29 CFR 1910.178(m)(3): A safe place to ride was not provided where riding a powered industrial truck(s) was authorized: Site- An employee was not protected from amputation, struck by, and fall hazards where the Crown SP 3400 Series, serial number 1A350125, order picker was operated with a Koke Inc., model OP-4896A-ERR, 8 foot platform attachment; the employee was a passenger working and riding on the platform attachment where there was no safe place to ride. The Crown SP 3400 Series owners manual does not allow passengers because there is no safe place for them.
