29 CFR  1910.178(a)(4):Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly:  Site- Employees were not protected from overloading, struck by, and fall hazards where the Crown SP 3400 Series, serial number 1A350125, order picker was operated with a Koke Inc., model OP-4896A-ERR,  8 foot platform attachment and did not have an identification plate which identified the capacity of the order picker with the attachment in place.  The platform attachment displayed a capacity of 1500 pounds on the side of the platform which exceeded the maximum capacity of the order picker with the platform attached.
