Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:  a) Motor Power Great Falls Inc, Steel Storage Rack "WG" : On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that were not securely fastened to the concrete floor.  b) Motor Power Great Falls Inc, Steel Storage Rack "WI" : On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that were not securely fastened to the concrete floor.  c) Motor Power Great Falls Inc, Steel Storage Rack "WJ" : On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that were not securely fastened to the concrete floor.  d) Motor Power Great Falls Inc, Steel Storage Rack "WL" : On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that were not securely fastened to the concrete floor.  e) Motor Power Great Falls Inc, Steel Storage Rack "WO" : On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that were not securely fastened to the concrete floor.  f) Motor Power Great Falls Inc, Steel Storage Rack "WP" : On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that were not securely fastened to the concrete floor.  g) Motor Power Great Falls Inc, Steel Storage Rack "WF" : On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that were not securely fastened to the concrete floor.  h) Motor Power Great Falls Inc, Steel Storage Rack "WC" : On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that were not securely fastened to the concrete floor.  i) Motor Power Great Falls Inc, Steel Storage Rack "WG": On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that did not display a plaque designating load sizes. The employer failed to identify the load rating of the storage racks in the warehouse to ensure they were not overloaded.  j) Motor Power Great Falls Inc, Steel Storage Rack "WI": On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that did not display a plaque designating load sizes. The employer failed to identify the load rating of the storage racks in the warehouse to ensure they were not overloaded.  k) Motor Power Great Falls Inc, Steel Storage Rack "WJ": On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that did not display a plaque designating load sizes. The employer failed to identify the load rating of the storage racks in the warehouse to ensure they were not overloaded.  l) Motor Power Great Falls Inc, Steel Storage Rack "WL": On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that did not display a plaque designating load sizes. The employer failed to identify and remove the damaged steel storage racking components.  r) Motor Power Great Falls Inc, Steel Storage Rack "WI": On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that contained damage. The employer failed to identify and remove the damaged steel storage racking components.  s) Motor Power Great Falls Inc, Steel Storage Rack "WC": On or about April 20, 2022 and at times prior, employees were exposed to struck by hazards while conducting tasks such as, but not limited to: loading and unloading repair parts for sale and installation from the storage racks that contained damage. The employer failed to identify and remove the damaged steel storage racking components.
