On or about April 21, 2022, at the address of 6120 N. Detroit Ave. Toledo, Ohio 43612: The employer did not ensure that the extension tips welded to forks met or exceeded manufacturer's approval prior to use. Operating the equipment in this condition may lead to struck-by injuries to operators, as well as other employees working in and around forklift operating areas.

29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: a) TNT Solid Solution LLC. On or about April 21, 2022, at the address of 6120 N. Detroit Ave. Toledo, Ohio 43612: The employer failed to ensure the employee furnished 4.5 inch angle grinders were inspected for safe operating conditions prior to employee use. Employees using the grinders without the factory equipped guards were exposed to struck-by hazards, lacerations, amputation injuries.
