On or about April 21, 2022, at the above address, the employer failed to ensure the employee who operate power industrial trucks including, Cat lift truck, Model 2P5000, Serial AT3531076 was reevaluated and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards.
