OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees exposed to struck-by hazard: On or about April 21, 2022 and times prior thereto, at the site the structural integrity of storage racks had become compromised from: 1) footing not secured to the floor; 2) no plaques with legible maximum permissible unit load and/or maximum uniformly distributed load per level. Employees were exposed to a struck-by hazard from the collapse of the storage racks and/or falling materials.

29 CFR 1910.305(g)(2):Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws: On or about April 21, 2022 at the site in the truck bed department employees were exposed to electrical shock hazards when operating the pendant controls of the the 1 Ton overhead crane next to the robotic welder when strain relief was not provided.
