29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level.  On or about April 1, 2022, and times prior thereto, an employee was working at a height of approximately 10 feet on the second floor of an apartment building under construction. The employee was utilizing fall protection but it did not prevent him from falling more than 6 feet, exposing the employee to a fall hazard.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about April 1, 2022, and times prior thereto, employees were performing framing work on a  residential apartment building without being trained and certified for fall protection, exposing employees to the hazard of falling approximately 10 feet to a lower level.
