29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) On or about April 21, 2022, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about April 21, 2022, at the above addressed jobsite, an employee conducting roofing operations was not protected from falling more than six (6) feet, thereby exposing the employee to an approximate ten (10) feet, six (6) inches fall hazard.
