The platform width was greater than the width of the truck (measured across the load bearing tires, plus 10 inches on either side), on or about April 21, 2022.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

A manufacturer's WARNING label posted on the outside of the operators cab door of a JLG/Gradall G6-42A Material Handler used to transport materials and elevate employees was observed in damaged condition with the instructions unreadable, on or about April 21, 2022.  Abatement certification required within 10 days after abatement date.
