29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.  a) Empire Marble and Granite, LLC./5 Chris Court/Dayton, NJ 08870 - Facility & Work Sites - Employees were exposed to struck by hazards while granite was being transported by a Mitsubishi forklift utilizing an unapproved Aardwolf Lifting Clamp (Serial#002981) on the boom attachment. The forklift truck's data and capacity plates were not changed to reflect the clamp attachment.   Condition noted on or about 4/21/2022.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  a) Empire Marble and Granite, LLC./5 Chris Crt./Dayton, NJ 08870 - Facility & Work Sites - Forklift operators were not competent to operate powered industrial trucks safely by completing the training and evaluation specified in 29 CFR 1910.178(l)(1). Company employees assisted in forklift operations by steadying the granite on the forks and standing nearby. Employees would thus be exposed to struck by hazards from improper forklift operation.   Condition noted on or about 4/21/2022.
