29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed:  a) On or about April 21, 2022, and at times prior, Key Structures, LLC., did not ensure that unsafe conditions disclosed during the periodic inspections on February 1, 2022, were corrected before operation of the cranes was resumed exposing employees to struck-by and crushing hazards by overhead gantry cranes in the facility as follows:  1) North Roofing Crane #5, S/N 20201: Bridge lights not working, and safety chain was off.  1/2" gap at rail splice. West gear box is not mounted correctly, small leak. southwest bumper completely breaking apart. All rail sweeps need safety cable. No Anti-Collision siren or slow down when cranes get to close to each other. 13 TON on bridge, only one hoist block that is 6.5 Ton. bridge needs to be downgraded to 6.5 Ton to avoid confusion. Old mag disconnect bracket are bent to the north angle is cracked. No beacon light/siren when operating. Northside not being used no rope on drum. Three resistors getting hot melting epoxy on them. Missing locking pin, hook latch bent. Rope needs change ASAP over 10 bad kinks. Cannot read capacity signs on block. Festoon needs a main festoon tow trolley. Gleason reel not hooked up, exposed wires. All rail sweeps need safety cables.  2) Roof Crane #4, S/N 2040950C: No on/off light. Pendant festoon rail is bent. Southwest bumper broke, breaking apart, no safety cables. Southwest & Southeast both bent down and loose. Southeast sweep bent and loose. Compass sticker incorrect needs new one, 5 TON hoist on a 10 TON bridge. No Anti-Collision siren when cranes get close to each other. Rope two small kinks. No Beacon light/siren when operating. Wheel gap from bridge flange is a 1/2" over recommended gap size.   3) Roof Crane #3, S/N 05A21661: End Trucks Northeast & Northwest both bent up. Bumpers Northwest rubber broken off Southwest & Southeast both bent down and loose. Southeast rail sweep bent up. Compass sticker under girder missing, needs grease. Latch broken no locking pin. Rope looks like it is unwinding from it sell, need replaced. No Beacon light/siren when operating. No anti-collision siren/slow down when cranes get to close.

29 CFR  1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  a) On or about April 21, 2022, and at times prior, Key Structures, LLC., did not ensure that damaged flexible cord sets that could have exposed employees to injury were removed from service. Employees in the framing department were exposed to contact with electrical hazards while using a Skilsaw connected to an extension cord that had been damaged and spliced.  The extension cord was not removed from service.

29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems are rigged in such a manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level:    a) On or about April 21, 2022, Key Structures, LLC., as the controlling and the exposing employer, did not ensure that personal fall arrests systems were rigged in such a manner that the employee could not free fall more than 6 feet or contact a lower level. Employees of Petrali Roofing, a subcontractor, and an employee of Key Structures, LLC. ,were exposed to fall hazards greater than 11 feet. The lanyard on their fall arrest system was tied off to a scissor lift adjacent to the manufactured home where they were applying a layer of rubber roofing material. The fall arrest system was rigged in such a manner that in case of a fall the employees would have contacted the concrete floor below.     Key Structures, LLC., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1515993, citation number 1, item number 4, and was affirmed as a final order on September 3, 2021, with respect to a workplace located at 319 Lockheed St. Pueblo, CO 81001.

29 CFR  1904.32(a)(1): The employer did not review the OSHA 300 log to verify that entries were complete and accurate, and correct any deficiencies identified:   a) On or about April 21, 2022, and at times prior, Key Structures, LLC., did not ensure that Injury and Illness logs, forms 300 and 300 A for year 2020, were complete and accurate. The employer did not provide forms that included total number of hours worked. The deficiencies were not identified and corrected.
