Located at 20 Erin Hills Road in Commercial Point, Ohio: On or about April 22, 2022, employees were performing roofing work with a full body fall arrest harness attached to rope lifelines that were wrapped around the ridge and nail plate of the roof trusses. The personal fall arrest systems in use did not have adequate anchorage, thereby exposing employees to a fall hazard.

Jose Juan Osorio Morales was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1390338, citation number 1, item number 1a and was affirmed as a final order on 05/03/2019, with respect to a workplace located at 618 Ludham Trail, Lot 178, Pickerington, Ohio 43147.
