29 CFR 1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent persons designated by the employers. (a) At 21 Midland Park Drive, O'Fallon, Missouri 63366 - on or about April 22, 2022, Westhaven Builders LLC, the controlling employer, did not conduct an adequate inspection of the job site, materials, and equipment. Subcontractor employees were exposed to the hazard of impalement while working near and around unguarded steel pins.
