29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about 4/22/2022, the employer did not provide door interlocks or other methods for shutting down the primer machines (listed below) when doors are open. The machines have rotating mechanisms while in operation. a) Machine #4 - 2015 Alpha Loading Systems Priming Machine, S/N 453 b) Machine #5 - 2016 Alpha Loading Systems Priming Machine, S/N 484

29 CFR 1904.39(a)(2):The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. On or about 04/22/222, at the facility, a work-related hospitalization was not reported to OSHA within 24-hours after an employee was injured while working on a primer machine.
